New Client Contact Form: If you are a new customer to Air Park, expedite your checkin by printing and completing this form prior to your first visit.

Wellness Checklist for Senior Cats: If your cat is 7 years or older please fill out this form and bring it for the doctor at your pet’s next appointment.

Wellness Checklist for Senior Dogs: If your dog is 7 years or older please fill out this form and bring it for the doctor at your pet’s next appointment.

Dental Grading Chart: A good indicator of your pet’s overall health is it’s teeth’s condition. Our technicians and doctors use this chart during your pet’s wellness exams. Feel free to print it and bring it the next time you come in to compare your
rating to ours.

FROM BALTIMORE, MD:
From I-695, Take the I-795, Exit 19, towards Owings Mills/Reisterstown.
Merge RIGHT onto I-795 N. to end. Merge onto RT. 140 towards Westminster.
Proceed on RT. 140 through Westminster.
Take the MD-97 NORTH ramp towards Union Mills/Gettysburg.
Merge RIGHT onto MD-97.
Parking is available in front of the bldg.
FROM WASHINGTON, D.C.:
From I-495, Take RT 270 North to MD RT. 27.
Take RT. 27 to Westminster.
Turn LEFT on Main Street in Westminster.
Turn RIGHT onto Pennsylvania Ave (RT 97).
Proceed on MD-97 towards Union Mills/Gettysburg.
Parking is available in front of the bldg.
FROM PENNSYLVANIA:
From Littlestown, PA, Take RT 97 South toward Westminster.
